The Mount Sinai School-Based Health Center at P.S. 83 provides free medical and mental health services to all enrolled students, right in our school building.

This clinic works together with your child's regular doctor to keep students healthy, learning, and in school.

What the clinic can do for your child

Mount Sinai's school-based health centers offer comprehensive primary care, mental health care, and preventive services for students in East Harlem public schools.

At P.S. 83, students can receive care for everyday health needs as well as support for managing ongoing conditions.

Services at the clinic

Students can receive a full range of care during the school day, from check-ups to counseling.

Medical care (right at school)

Complete physical exams (including annual check-ups and school health forms).
Care for common illnesses (colds, flu, stomach bugs, asthma flare-ups, minor injuries).
Ongoing care for chronic conditions (such as asthma or allergies), coordinated with your child's primary care doctor.
First aid during the school day.
Immunizations required for school and recommended vaccines.
Prescriptions and dispensing of common medications when needed.
Laboratory testing (such as blood work, urine tests, strep tests, and other routine tests).

Forms and school participation

Medical forms for after-school programs and clubs.
Medical forms for sports and physical activity.
Medical forms for summer camp and other youth programs.

Mental and behavioral health support

Individual counseling for stress, anxiety, sadness, behavior concerns, and family changes.
Support with peer relationships, bullying, and school adjustment.
Referral to additional mental health services when needed.

Health education and prevention

One-on-one health education to help students understand their health conditions and treatments.
Guidance on healthy habits, such as nutrition, sleep, physical activity, and hygiene.
Support for families with questions about their child's health and development.

How much does it cost?

All services at the Mount Sinai School-Based Health Center are provided at no cost to families, regardless of insurance or ability to pay.

If your child has health insurance, the clinic may bill the insurance, but there are no co-pays or out-of-pocket costs for families.

Does this replace my child's regular doctor?

No. The School-Based Health Center does not replace your child's primary care physician.

Instead, it adds an extra layer of convenient care during the school day and can share information (with your permission) to coordinate with your child's doctor.

Who can use the clinic?

All P.S. 83 students are eligible to enroll in the Mount Sinai School-Based Health Center.

Students must be registered to receive full services, but any student can receive first aid and emergency care.

How to enroll your child

To give your child access to all services, please complete and return the Mount Sinai School-Based Health Center consent and registration forms.

1Get the consent forms from the clinic office (Room 109), the main office, or the downloadable packet on the Mount Sinai School Health Program website.
2Review and sign all sections of the forms.
3Return the forms to your child's teacher, the main office, or directly to the clinic in Room 109.
4Once enrolled, your child can be seen during the school day as needed.

Families have 24-hour access to a Mount Sinai physician on call for urgent questions or concerns related to the clinic's care by phone.

Who works in the clinic?

Mount Sinai's school-based health centers are staffed by a team that may include pediatricians, nurse practitioners, and physician assistants; social workers and mental health clinicians; and medical assistants and health educators. At P.S. 83, the on-site doctor is Doctor Perez.
